The SDOH Academy invites staff from Federally Qualified Health Centers (FQHCs), FQHC Look-Alikes (LALs), Primary Care Associations (PCAs), Health Center Controlled Networks (HCCNs), and their partner organizations to participate in the 2025 SDOH Academy Innovations Showcase Competition: 

Fostering a Healthy Future for Adolescents and Young Adults

This interactive training experience offers health centers a unique opportunity to learn about SDOH and showcase their innovative approaches. Participants will have the chance to:

  1. Attend a live kick-off webinar featuring dynamic guest speakers and SDOH Academy Faculty.

  2. Access three on-demand micro-training modules, earning a certificate of completion that demonstrates foundational knowledge of youth-specific social drivers of health (SDOH) and Healthy People 2030 goals.

  3. Submit an abstract for a chance to be featured on the live closing webinar before a national audience to showcase outstanding innovations that promote a healthy future for adolescents and young adults. This is an excellent opportunity to learn how multidisciplinary healthcare teams address unmet needs while tackling SDOH.

How to participate?

  • Watch the recording of the SDOH Academy Innovations Showcase Kick-Off Webinar hosted on February 12, 2025  at 3 - 4:30 PM (ET) to learn more about this innovations showcase competition.

  • Complete three pre-recorded micro-training modules at your own pace, but by no later than April 16, 2025 by 6PM ET. To be eligible to compete in the showcase, you are required to complete the learning modules.

  • Submit your innovation abstract using the submission form by April 16, 2025 by 6PM ET. Your innovation can be in the form of a video, poster, or brief document.

  • Join the SDOH Innovations Showcase Closing Webinar on May 21, 2025, at 3-4:30 PM ET (you will be automatically registered once you register for the kick-off webinar and will receive the link closer to the webinar date )
