Childhood chronic diseases are on the rise—threatening the health and future of the next generation. The Connected Communities Health Initiative is launching a national webinar series to help Health Centers respond to this urgent crisis.
Through engaging webinar sessions, staff of Health Centers, PCAs, and HCCNs will gain the knowledge, tools, and strategies needed to address the root causes of childhood chronic disease. Sessions will explore prevention, environmental and dietary risk factors, behavioral health in the digital age, medication overuse, and the role of data and partnerships in driving change.

Participants will learn how to:
Understand the latest trends in childhood chronic disease impacting health center patients
Apply research findings to quality improvement, population health management, and integrated care
Use actionable tools, screenings, and data strategies to strengthen prevention and care in primary care practice
Leverage PCA and HCCN support to scale innovations across the health center network
