Building Community Partnerships to

In 2019, we hosted a learning collaborative to provide health center staff with tools and strategies to initiate, develop, and sustain community partnerships to better serve older adult residents of public housing. Those trainings are available below. The learning collaborative was led by the National Nurse-Led Care Consortium, the National Center for Equitable Care for Elders, and the National Center for Health in Public Housing.
