Using an innovative "ground up" public health approach, the program is designed to educate healthcare workers, educational workers, faith-based organizations, and families in communities in North Texas about SDOH. The curriculum has proven to be very successful at improving knowledge about Adverse Childhood Experiences, SDOH, and community response to these.

Hui Hoaka (HH), which translates to "team abundance", was a new arm of the organization focusing on community resilience at the height of the pandemic. Created in 2020, HH’s immediate goal was to keep the community safe through equitable access to culturally grounded care, information, and resources. The HH team provides wrap around care for patients, ensuring they have sufficient resources to address the challenges brought by the pandemic.

For the past five months, the health center has been working with 4 DPI teams to improve access to care and address SDOHs. These teams have established best practices for decreasing Cycle Time, decreasing Missed Opportunities, and improving timely Charting Completion.

The goal is to increase awareness of how social determinants of health are an integral part of treating patients holistically. This will be done by creating a uniform screening tool to be integrated in its clinics to address these issues. A combination of the PRAPARE tool and the UNITE US platform will be used to collect information as well as track them in real-time.

The program was launched in 2021 with the establishment of a Community Fridge at the Health Center. To aid residents who may lack financial stability, the fridge was put in place to provide free, healthy foods to a community that additionally lacks affordable grocery options and access to nutritious produce. The Community Fridge is run by the Food Insecurity Program and individuals who volunteer when the Health Center is closed.

A Promise of Hope is a community-based collaboration between AlohaCare and the Waipahu Community School for Adults to benefit education and health outcomes in Hawai'i. This innovation is designed to: a) address health, educational, economic, social, and cultural issues that face underserved communities in Hawai‘i; b) engage adults living in rural and medically underserved neighborhoods in SDOH services; and c) support Medicaid enrollees to achieve educational attainment of high school equivalency through comprehensive support services.

In 2013, the Center's team implemented a PN intervention for breast and cervical cancer tailored to an underserved U.S. Chinese immigrant community marked by low screening rates, culturally embedded stigma, and a healthcare system with few and fragmented options for linguistically isolated patients. The PNs tackled myriad Social Determinants of Health (SDOH) barriers and served as an indispensable tool to meet the needs of this population.

Grapefruit Health recruits and trains clinical students to perform interactions with specific patient populations. The students get an incredible opportunity to learn while they earn income and Grapefruit Health's partners are able to do more for the community at a fraction of the cost. Most importantly, the patients are getting the outreach they need, and clinical programs now have a way to better train their students.
